Lowland Care is working in partnership with Dumfries and Galloway Council, Association for Real Change Scotland (ARCS), Loreburn Housing Association (LHA) and Helen Sanderson Associates (HSA) to pilot person-centered practice in a home care setting, using person-centred assessment and related tools to completely redefine home care delivery and align it to individuals’ aspirations using self directed support.

 The partnership is a co-creative process rooted in person-centred thinking  connecting commissioners, providers, and specialists to involve service users and staff in creating a home care service that is truly person-centred, tailored to individual needs and aspirations, and underpinned by self directed support.

This pilot is the first of its kind in the UK; Striving to weave person-centered practice and thinking into the design and delivery of a home care service, from initial assessment by social work staff to service delivery by home carers. The programme ensures that each person supported through a home care package will be able to determine the nature and specification of their support starting with assessment by social work staff to delivery and monitoring outcomes by home care staff and commissioners.

 Practical person-centred tools are used to determine specific elements in each persons home care and support package. This is then translated into actions for home carers.
